A 67-year-old male with chest pain, headache and double vision with a history of latent syphilis presented with multiple rim-enhancing brain lesions on MRI. A Gram stain of a brain biopsy shows beaded gram-positive rods, and a Kinyoun stain shows acid-fast bacilli in the brain.
